Medical language, something more than information. (On how linguists and physicians can understand each other)
La presencia en el lenguaje de un importante componente de poder nos lleva necesariamente a la consideración de la dimensión ética implícita en las tareas que se repiten en cada uno de los actos verbales en que participamos. Cualquier especialista en un ámbito del saber debe ser consciente de cómo funciona el lenguaje en muchos aspectos que directamente le afectan, de cómo la selección de los elementos y su combinación —estén éstas más o menos automatizadas— han de ir dirigidas a la adecuación de un mensaje que no desoriente a su interlocutor.
En este artículo se propone una reflexión sobre unas tareas que no se llevan a cabo de una manera ingenua, sobre unos dominios en los que la palabra, además de servir para comunicar algo, tiene dos poderes: el de la acción y también el de la sugestión. Todo un mundo de hechos explícitos e implícitos entra en juego cada vez que alguien se decide a actuar por medio del lenguaje. Un mundo de intenciones y expectativas, de colaboración y compromisos, al que el médico no puede dar la espalda.The fact that language is comprised of an important power component brings us, of necessity, to reflect upon the ethical dimensions of the tasks involved in each and every verbal act we engage in. Specialists in any field should be aware of how language works in many ways that directly affect them, of how the choice of elements and their combination—regardless of how mechanized they may be—should be geared toward crafting a message that does not mislead the interlocutor.
This article puts forth ideas on certain tasks that are not performed naively, on domains in which words, besides conveying a message, have two types of power: the power of action and the power of insinuation. An entire universe of explicit and implicit events comes into play whenever someone decides to act by making use of language. Linguistic and symbolic differences in the discourse of young Ecuadorians residing in Madrid.
Si bien una de las características de los colectivos sociopolíticos tiene que ver con el hecho de que en general se trata de grupos construidos en el sentido de que no son naturales (“imagined communities”, Anderson, 1983), las diferencias observadas entre ellos les confieren al menos cierta ilusión de realidad, llegando incluso algunas de ellas a dibujar fronteras que se pueden interpretar de manera más o menos objetiva, es decir que inducen a concebir los colectivos asociados a ellas en términos naturales. Con todo, al lado de este tipo de fronteras que ignoran la diversidad inherente en cualquier grupo (Ambadiang, Palacios y García Parejo, 2008), cabe imaginar otras fronteras, de naturaleza menos estática y más acorde con la dinámica característica de las situaciones de contacto. De acuerdo con esta última perspectiva, nos proponemos indagar en la relevancia que tienen tanto las propias diferencias lingüísticas y culturales como su uso en la construcción de la identidad individual y colectiva por parte de los jóvenes ecuatorianos residentes en un contexto muy específico de contacto: Madrid. A partir de la narración que se recoge en las respuestas abiertas en un cuestionario aplicado a 56 estudiantes ecuatorianos de Educación Secundaria intentamos determinar cómo construyen discursivamente su identidad social, así como el papel que desempeña la variación lingüística en este proceso. Nuestros datos sugieren que los procesos de categorización de los jóvenes ecuatorianos se distribuyen alrededor de tres estrategias básicas que pasan por considerar al colectivo español como: (i) una mera comparación; (ii) una fuente de categorizaciones, generalmente negativas; y (iii) una imagen especular.Although one of the defining features of socio-political groups is that they generally are imagined communities (Anderson, 1983), in the sense that they are not natural, the differences between them allow at least some illusion of reality, that in some of them provokes the raising of frontiers which may be interpreted more or less objectively, inasmuch as they may induce to conceive as natural the groups associated to them. Besides such frontiers, which lose sight of the diversity inherent to any group (Ambadiang, Palacios & García Parejo, forthc.), others may be thought of which are more dynamic and more in harmony with the processes which are typical of contact situations. qu’il s’agit en général de groupes imaginés dans ce sens qu’elles ne sont pas naturelles
(«imagined communities »; Anderson, 1983), les différences observées entre elles leur
attribuent à tout le moins une certaine illusion de réalité, qui donne lieu dans le cas de
certaines d’entre elles à l’érection de frontières qui peuvent être interprétées de manière
plus ou moins objective, dans la mesure où elles peuvent induire à concevoir comme
naturelles les collectivités associées à elles. Outre ces frontières qui perdent de vue la
diversité inhérente à tout groupe (Ambadiang, Palacios & García Parejo, 2008), il y a
lieu d’en imaginer d’autres, de nature plus dynamique et plus en accord avec les
dynamiques caractéristiques des situations de contact. De ce point de vue, nous nous
proposons d’étudier l’importance aussi bien des différences linguistiques et culturelles

des jeunes Équatoriens en résidence dans un contexte très précis : à savoir, la

recours les jeunes Équatoriens s’articulent autour de trois grandes stratégies qui

Un programa de intervención auto-aplicado a través de Internet para el tratamiento de la depresión: “Sonreír es divertido”
La depresión es una de las grandes problemáticas para la salud mundial en la actualidad. Es necesario entonces disponer de intervenciones eficaces para su tratamiento. Si bien en la actualidad existen numerosos tratamientos psicológicos con apoyo empírico sobre su eficacia, la población que se beneficia de los mismos es muy reducida, debido a los altos costes de su implementación, al tiempo requerido y a la falta de profesionales bien entrenados. El uso de Internet aparece como una herramienta útil para administrar los tratamientos y superar los problemas que se presentan. En este sentido, el presente trabajo tiene por objetivo presentar la descripción de un programa de intervención autoaplicado a través de Internet para el tratamiento de la depresión, desarrollado por el Laboratorio de Psicología y Tecnología (LabPsiTec)
Validación en castellano del Parental Acceptance Questionnaire (6-PAQ)
Background/Objective
Psychological flexibility is an ability with positive effects in health, also in the field parenting. The aim of this study is to validate the Parental Acceptance Questionnaire (6-PAQ) in Spanish sample.
Method
A total of 426 parents (96.3% female), with an average age of 38.25 years (SD = 5.76) and with at least one child between 3 and 16 years old participated. Participants completed the 6PAQ and other instruments assessing experiential avoidance, thought suppression, satisfaction with life and symptoms of anxiety and depression.
Results
In comparison with the original model, the three-factor structural solution with 16 items obtained a better adjustment (CFI = .996; GFI = .998; TLI = .995; RMSE = .016) that refer to response styles related to psychological flexibility (open, aware and active). The 6-PAQ adaptation has shown good internal consistency, with a Cronbach alpha of .81 and a McDonald's omega of .86 for the scale, and evidence of convergent validity with significant correlations with measure of avoidance, thought suppression, satisfaction with life and anxiety.
Conclusions
The Spanish version of the 6-PAQ with the three-factor structure presents psychometric property to be considered a reliable measure and a valid instrument for the evaluation of parental psychological flexibility.Antecedentes/Objetivo
La flexibilidad psicológica es una habilidad que tiene efectos positivos en la salud, y también en el ámbito de la parentalidad. El objetivo del estudio es validar el Parental Acceptance Questionnaire (6-PAQ) en población española.
Método
Participaron 426 padres (96,30% mujeres), con una media de edad de 38,25 años (DT = 5,76) y con al menos un hijo entre 3 y 16 años. Los participantes completaron el 6-PAQ y otros cuestionarios de evitación experiencial, supresión de pensamientos, satisfacción vital, y sintomatología de ansiedad y depresión.
Resultados
En comparación con el modelo original se obtiene un mejor ajuste para la estructura factorial de tres factores con 16 ítems (CFI = 0,996; GFI = 0,998; TLI = 0,995; RMSE = 0,016) que hacen referencia a estilos de respuesta relacionados con la flexibilidad psicológica (abierto, centrado y comprometido). Los resultados mostraron una buena consistencia interna, con una alfa de Cronbach de 0,81 y un omega de McDonald de 0,86, así como evidencias de validez convergente con correlaciones significativas con medidas de evitación, supresión de pensamientos, satisfacción vital y ansiedad.
Conclusiones
La versión española del 6-PAQ, con estructura de tres factores, presenta propiedades psicométricas para considerarse una medida fiable y un instrumento válido para la evaluación de la flexibilidad psicológica parental
The impact of chronic pain on patients and spouses: Consequences on occupational status, distribution of household chores and care‐giving burden.
Background: Informal care-giving by spouses has become frequent in chronic pain settings. However, the impact of pain on occupational, functional and health out-comes in spouses has not been systematically investigated.Aims: The goal of the present study was to examine the impact of pain on both pa-tient and spousal outcomes.Methods: In the present study, the impact of chronic pain on 114 heterosexual dyads was explored (patients: 59% females, average age=57.81years, SD=11.85; spouses: 41% females, average age=57.32years, SD=12.15).Results: Overall, both patients and spouses reported important consequences of pain on outcomes, including occupational status distribution of household chores and marital satisfaction). Almost 52% of spouses indicated a high-to-severe burden. A multivariate model with spouse and patient factors accounted for 37.8% of the vari-ance of this burden. In the model, patient disability (=0.36, p=.002), spouses’ change in occupational status (=0.26, p=.002) and spousal perception of marital adjustment (=−0.36, p<.001) were uniquely associated with burden.Conclusions: The results indicate that the impact of chronic pain should be evalu-ated both for patients and spouses and point to patient and spouse factors that might contribute to spousal burden, which might help guide family interventions in a more effective manner.Significance: Research has shown that chronic pain poses a significant burden on individuals, which increases their reliance on others for assistance. However, the bur-den of informal care-giving assumed by spouses of patients with chronic pain has not been systematically investigated. This study offers new insights into the impact of chronic pain on patients and their spouses, which might provide empirical foundation for the development of new avenues for intervention aimed at promoting adjustment in patients with chronic pain and spouses who act as informal caregivers
Long-term Effectiveness and Predictors of Transdiagnostic Internet-Delivered Cognitive Behavioral Therapy for Emotional Disorders in Specialized Care: Secondary Analysis of a Randomized Controlled Trial
Background: Transdiagnostic internet-delivered cognitive behavioral therapy (iCBT) for emotional disorders has been shown
to be effective in specialized care in the short term. However, less is known about its long-term effects in this specific setting. In
addition, predictors of long-term effectiveness may help to identify what treatments are more suitable for certain individuals.
Objective: This study aimed to analyze the long-term effectiveness of transdiagnostic iCBT compared with that of treatment
as usual (TAU) in specialized care and explore predictors of long-term effectiveness.
Methods: Mixed models were performed to analyze the long-term effectiveness and predictors of transdiagnostic iCBT (n=99)
versus TAU (n=101) in public specialized mental health care. Outcomes included symptoms of depression and anxiety, health-related
quality of life (QoL), behavioral inhibition and behavioral activation, comorbidity, and diagnostic status (ie, loss of principal
diagnosis) from baseline to 1-year follow-up. Sociodemographic characteristics (sex, age, and education) and clinical variables
(principal diagnosis, comorbidity, and symptom severity at baseline) were selected as predictors of long-term changes.
Results: Compared with baseline, transdiagnostic iCBT was more effective than TAU in improving symptoms of depression
(b=–4.16, SE 1.80, 95% CI –7.68 to –0.67), health-related QoL (b=7.63, SE 3.41, 95% CI 1.00-14.28), diagnostic status (b=–0.24,
SE 0.09, 95% CI –1.00 to –0.15), and comorbidity at 1-year follow-up (b=–0.58, SE 0.22, 95% CI –1.00 to –0.15). From
pretreatment assessment to follow-up, anxiety symptoms improved in both transdiagnostic iCBT and TAU groups, but no
significant differences were found between the groups. Regarding the predictors of the long-term effectiveness of transdiagnostic
iCBT compared with that of TAU, higher health-related QoL at follow-up was predicted by a baseline diagnosis of anxiety, male
sex, and the use of psychiatric medication; fewer comorbid disorders at follow-up were predicted by older age and higher baseline
scores on health-related QoL; and fewer depressive symptoms at follow-up were predicted by baseline diagnosis of depression.
However, this pattern was not observed for baseline anxiety diagnoses and anxiety symptoms.
Conclusions: The results suggest that transdiagnostic iCBT is more effective than TAU to target depressive symptoms among
patients with emotional disorders. Anxiety symptoms remained stable at 1-year follow-up, with no differences between the groups.
Results on predictors suggest that some groups of patients may obtain specific gains after transdiagnostic iCBT. Specifically, and
consistent with the literature, patients with baseline depression improved their depression scores at follow-up. However, this
pattern was not found for baseline anxiety disorders. More studies on the predictor role of sociodemographic and clinical variable
La interferencia de la utilización concurrente de Alprazolam en el tratamiento cognitivo-comportamental del trastorno de pánico
This study analyzes the role of alprazolam in the therapeutic effectiveness of cognitivebehavioral treatments for panic disorder. This work belongs to a wider study which aim was to study the differential effectiveness of two programs, a standard one and a brief one supported by self-help manuals. We demonstrated the effectiveness of both interventions. The aim of the present study is to analyze the differential effectiveness according to the use of alprazolam. The results shows that the patients who used alprazolam improve as well as patients that didn’t use it in mostly of the analyzed variables at postest and follow-up assessment (12-month follow-up)El presente estudio analiza el papel de la utilización concurrente del alprazolam en la eficacia de la terapia cognitivo-comportamental para el trastorno de pánico. El trabajo se engloba dentro de un estudio previo más amplio. El objetivo de dicho estudio fue estudiar la eficacia diferencial de dos tratamientos, uno estándar y otro abreviado apoyado por manuales de autoayuda. Los resultados indicaron que ambos tratamientos eran igualmente eficaces. El objetivo del presente estudio es analizar la eficacia diferencial en función de si los pacientes tomaban o no alprazolam. Los resultados indican que, en prácticamente la totalidad de las variables analizadas, los sujetos mejoran por igual en el postest y en el seguimiento (a los doce meses de finalizada la terapia), independientemente de si empezaron el programa cognitivo-comportamental tomando o no un tratamiento farmacológico (alprazolam)
Psychological intervention using virtual reality for patients with cancer.
El objetivo de este trabajo es presentar las principales aportaciones de la Realidad Virtual (RV) en el tratamiento psicológico con pacientes oncológicos, y describir un programa de intervención que utiliza esta tecnología para promover bienestar emocional en pacientes hospitalizados. La RV se presenta como una prometedora herramienta de apoyo al paciente oncológico. Diversos estudios plantean su utilidad para hacer algunos tratamientos médicos más tolerables, para reducir los niveles de distrés y dolor, así como promover mayores niveles de emociones positivas. Se ha empleado principalmente con pacientes ambulatorios, existiendo una menor proporción de propuestas dirigidas al período de hospitalización. La intervención que ha desarrollado nuestro grupo aborda esta condición desde un foco también poco explorado: la promoción de bienestar. Esta intervención se compone de 4 sesiones (módulos de distracción y reminiscencia) implementadas individualmente en la habitación del paciente. Antes y después de cada sesión se evaluó el estado emocional (escalas visuales), la percepción de paso del tiempo (en minutos), de cambio en el estado de ánimo y de satisfacción con el procedimiento. Los resultados iniciales señalan incrementos en las emociones positivas después de cada sesión, existiendo cambios significativos en la primera y tercera sesión. Además, la mayoría de pacientes informó que su estado de ánimo había mejorado después de las sesiones. A partir de estos hallazgos se observa que los entornos virtuales y las tecnologías de la información y la comunicación muestran utilidad no sólo para disminuir el malestar sino también para promover bienestar emocional, especialmente durante períodos de tratamiento y hospitalización.The purpose of the present study is to review the main contributions of Virtual Reality (VR) to the field of psychological treatment with cancer patients, and to describe an intervention that uses this technology to promote emotional well-being on inpatients. VR appears as a promising tool for supporting psychological treatments in cancer patients. Several studies explore its utility for promoting more tolerable medical treatments, lowering the level of distress and pain, as well as increasing positive emotions. VR has been mainly used with outpatients, with a minor proportion of proposals oriented to the hospitalization period. The intervention developed by our group addresses this issue from a focus not very much explored: the promotion of well-being. This intervention is composed of 4 sessions (distraction and reminiscence modules) implemented individually in the patients’ room. Emotional state (visual analog scales), perceived time duration (minutes), perception of mood change and satisfaction with the procedure were assessed before and after sessions. Preliminary results showed an increase in positive emotions after each session, being these changes significant in the first and third session. Besides, most patients perceived themselves in a better mood after the sessions. Virtual environments and information and communication technologies could be useful not only to diminish discomfort but also to promote emotional well-being, especially during medical procedures and hospitalizatio
“Family Connections”, a DBT-Based Program for Relatives of People with Borderline Personality Disorder during the COVID-19 Pandemic: A Focus Group Study
The COVID-19 pandemic has had a significant impact on the family environment due to
the difficulties that have been generated by job losses, deaths, increase rates of family and domestic
violence, poor mental health outcomes, and estrangement in personal relationships. “Family Connections” (FC) is an internationally renowned DBT-based program that supports the families and
caregivers of people with borderline personality disorder. The study took place at a Specialized Health
Centre in Spain. A focus group with seven participants was organized for people who had previously
attended an FC group. The participants were asked about their experiences during the confinement
periods that was caused by COVID-19 as well as their experiences and opinions on relatives, skills
practiced, their need to and the advantages of attending the group, and satisfaction with the FC group.
The qualitative research web program Dedoose was used for the thematic analysis of the data. The
results showed that the participants experienced various experiences during confinement; validation
and radical acceptance were determined to be the most useful skills; the importance of professionals
and the content as well as the sincerity of attendees and having a safe space were determined to be the
greatest benefits of the programs; and the participants all indicated great satisfaction of the program.
This study allowed us to explore the experiences of family members of people with BPD with their
loved ones during the confinement period caused by the COVID-19 pandemic. We evaluated the
